The useful diagnosis is that code production is accelerating faster than organizational ownership—and your fix is concrete: one accountable reviewer per code surface, written into CODEOWNERS, so exactly one name survives the PR volume. Paying for AI twice (tokens, then the coordination tax of figuring out who owns the bad merge) is the failure mode when three teams optimize the same workflow with no end-to-end owner. If a team can only install one of your four decisions first, which do you force: single named reviewer per surface, explicit EM code-contribution bounds, orchestration counted in engineer performance, or rebuilding the EM calendar around judgment agents still miss?
